This is a form completed by project leaders and project team members to apply for visiting SPring-8\/SACLA. This form is also used to apply for the use of the SPring-8 Guest House. Please make sure to submit this form at least 10 days prior to your arrival.
We will contact you after submission of the form<\/strong>. If you do not receive our e-mail, please check your spam folder. If you still can’t find it, please contact us.

How to submit<\/strong>
<\/h2>\n\n\n\n

Log in to My Page and submit an Application Form for SPring-8 Facilities online. On this form, provide the expected receive\/return dates of your user card and dosimeter and the expected SPring-8 Guest House check-in\/check-out dates. When the reservation has already been made for another proposal or there is no need for reservation, check the box for \u201cNO NEED FOR SPRING-8 GUEST HOUSE RESERVATION\u201d. If you don’t participate in the experiment, please select “Absence”. If multiple experiments are scheduled for the same period, an application form(Form 10) must be submitted for each experiment. You need to submit 3 forms, that is, Application Form for SPring-8 Facilities, List of Samples, Reagents, etc., and List of Carry-In Items(if you are planning to bring in any items), prior to your arrival.<\/p>\n\n\n\n

Please note that the links to List of Samples, Reagents, etc. (Form9) and List of Carry-In Items(Form8) are not displayed until after Application Form for SPring-8 Facilities(Form10) has been submitted.<\/p>\n\n\n\n

Making changes after submitting application form<\/h2>\n\n\n\n

If any changes occur after submitting the application form, please contact Users Office by email as changes cannot be made online. In case of adding participants to the experiment, please inform us of their names and schedules after the project leader has added them to the list of Project Team Members<\/a>.
